If you have experienced an incident, received treatment, or have a concern you believe should be reported, please complete the form below. Your feedback is important to us and helps ensure we continue to provide professional, accountable service to our community.

If you require immediate assistance or need to speak with someone directly, please contact the on-duty officer at 940-479-2488 to initiate the process.

All reports are reviewed thoroughly and handled in accordance with the policies and procedures of the Ponder Volunteer Fire Department. When required, matters will be referred to the appropriate authorities to ensure transparency, accountability, and proper resolution.

Complaint Procedure

An individual or a representative of an organization can file a complaint to report if a law or rule was broken by a community health worker, instructor, or training program as stated in Chapter 48, Texas Health and Safety Code or 25 Texas Administrative Code, §§146.1–146.8. The complaint is sent to the Department of State Health Services (DSHS). 

Note: This complaint form is specific to DSHS-CHW program oversight and support.
